Prevent duplicate submission reopen requests

An error was triggered by an attempt to review a Monthly Report reopen request, caused by two reopen request records existing for a single Monthly Report submission.

The two reopen request records were created only 5 seconds apart from one another, suggesting two consecutive reopen form submissions (such as via a re-click of the Reopen button) as the potential cause of the duplication.

While this particular instance of the bug has been resolved, a fix should be implemented to prevent future duplicate reopen requests for any Monthly Report, Claim, or Budget Filing submission.
