Statewide uploads to enforce using "SW" in the program ID

This is part of 2753

Statewide example files will be added to the Statewide upload page

  1. Claim/Site/Contact ID fields must start with "SW" instead of the PA code.

  2. Only statewide program IDs are allowed to be submitted (they also start with "SW")
